Job title: Speech Language Pathologist
Schedule: Full-time, 80 hours per pay period; 8:00am - 4:30pm
Weekend rotation: None required

Position Summary:
The Speech-Language Pathologist (SLP) evaluates, develops, and implements individualized treatment plans for residents in a Skilled Nursing Facility (SNF) with communication, cognitive-linguistic, voice, and swallowing disorders. The SLP works collaboratively with the interdisciplinary care team to maximize residents' functional independence, safety, quality of life, and ability to participate in daily activities.
Position Responsibilities:
- Evaluate and treat residents with speech, language, cognitive, voice, and swallowing disorders.
- Complete evaluations, treatment plans, progress notes, and discharge documentation.
- Assess and manage swallowing disorders, including diet and safety recommendations.
- Develop and update individualized treatment plans based on resident needs and goals.
- Participate in care conferences and interdisciplinary team meetings.
- Collaborate with physicians, nursing staff, therapists, dietitians, and other healthcare team members.
- Educate residents, families, and staff on treatment recommendations and swallowing precautions.
- Provide in-service training and education to facility staff.
- Monitor resident progress and adjust treatment plans as needed.
- Support discharge planning and transitions of care.
- Maintain compliance with facility policies, regulatory requirements, and documentation standards.
- Maintain treatment equipment and follow infection control practices.
- Perform other duties as assigned.
Position Requirements:
- Professional degree in speech language pathology required.
- Masters degree in speech pathology from an accredited program.
- Eligible for Speech-Language Pathologist license to practice in the state of WI.
- Must hold and maintain Wisconsin Speech-Language Pathologist license.
- Must maintain a valid, current driver's license.
- Must maintain BLS certification.
- 1+ years of related work experience preferred. Experience in a hospital setting a plus.
- Exceptional accuracy and attention to detail required.

- Intermediate proficiency with computers is required.
- Strong assessment and planning skills.
- Strong quantitative and analytical competency.
- Self-starter with excellent interpersonal communication and problem-solving skills.
