Job Title: CNA LTC

Contract Duration: 13 weeks

Location: Brooklyn, NY

Shift: Evening (3P-11P) AND Night (11P-7A)

Job Description:

• Assist residents with bathing, grooming, oral care, dressing, toileting, and feeding.
• Monitor and record vital signs, intake/output, bowel movements, weights, and resident conditions.
• Provide incontinent care, skin care, range-of-motion exercises, and reposition residents every 2–3 hours.
• Assist with transfers, ambulation, admissions, discharges, and transportation to appointments/activities.
• Observe and report changes in residents’ physical or mental condition to nursing staff.
• Maintain a clean, safe environment and complete required documentation.
• Respond promptly to call lights, alarms, and resident needs.
• Support residents with special care needs, including terminally ill, cognitively impaired, or isolated individuals.
• Perform resident monitoring (Q15/Q30 checks) and other duties as assigned by nursing leadership.

Requirements:

• High School diploma or equivalent (GED).
• New York State CNA License
• BLS
• Recent 1 year Epic EMR Experience
