Schedule: Full-Time Contract

Position Overview

"Navitas Healthcare, LLC" is seeking an experienced Licensed Clinical Social Worker (LCSW) to support patients in an acute care hospital setting. The ideal candidate will have experience with inpatient care, case management, care coordination, and interdisciplinary collaboration.

Key Responsibilities

• Provide psychosocial assessments and care planning

• Coordinate discharge planning and community resources

• Support patients and families through hospitalization and transitions of care

• Collaborate with physicians, nurses, and interdisciplinary teams

• Manage complex inpatient case management needs

• Maintain accurate documentation in EPIC

• Assist with care coordination for neurological and cardiac patient populations

• Ensure compliance with hospital and regulatory standards

Required Qualifications

• Active LSWAIC License

• BLS Certification

• Minimum 1 year of LCSW experience

• Acute Care / Inpatient Hospital experience required

• Case Management experience required

• EPIC experience required

• Strong communication and care coordination skills

Preferred Experience

• Stroke or Neuro patient population experience

• Cardiac patient care coordination experience

• Comprehensive discharge planning experience
