About the Clinic
The Community Medical Clinic of Kershaw County is a community-based free medical clinic dedicated to providing accessible, high-quality healthcare to underserved populations regardless of their ability to pay. Our mission is to promote health equity and wellness through compassion, education, and collaboration. Current service area is Kershaw County and portions of Lancaster County.
Position Summary
The LPN plays a vital role in supporting the free medical clinic's mission by providing compassionate, high-quality nursing care to uninsured and underserved patients. Working under the supervision of the Clinic Provider, the LPN assist with patient intake, assessment, education, medication management, and coordination of care in a community-based primary care setting. This role emphasizes patient-centered care, health education, disease prevention, and chronic disease management. Work settings may include main clinic, satellite locations and school-based health centers. Clinic is open Monday through Thursday, 8:00 am – 5:00 pm.
Key Responsibilities
· Obtain and document patient medical histories, vital signs, and other clinical information accurately and efficiently.
· Assist providers during patient examinations and procedures.
· Administer medications, injections, immunizations, and treatments as authorized and appropriate.
· Provide patient education in group or individual environment regarding medications, chronic disease management, preventive care, and healthy lifestyle choices.
· Maintain accurate and timely documentation in the electronic medical record system.
· Coordinate patient referrals, diagnostic testing, and follow-up appointments.
· Support care coordination and case management efforts for patients with chronic or complex medical conditions.
· Monitor and maintain clinical supplies, equipment, and medication inventory.
· Ensure compliance with clinic policies, infection control standards, safety procedures, and HIPAA regulations.
- Maintain accurate and timely documentation in EPIC, electronic health record (EHR).
- Support clinic outreach, health fairs, and patient education sessions as needed.
Qualifications
· Current and unrestricted Licensed Practical Nurse (LPN) license in the State of South Carolina.
· Graduate of an accredited practical nursing program.
· Minimum of one to three years of nursing experience preferred, especially in primary care, community health, family practice, or underserved healthcare settings.
· Current CPR/BLS certification required.
· Strong clinical assessment and patient care skills.
· Ability to accurately document patient information in an electronic medical record (EMR) system.
· Knowledge of infection control, medication administration, and patient safety standards.
· Excellent communication and interpersonal skills with the ability to interact compassionately and professionally with patients, volunteers, providers, and staff.
· Ability to work effectively in a fast-paced, team-oriented environment.
· Demonstrated sensitivity to the needs of uninsured, underserved, and culturally diverse populations.
· Strong organizational skills and attention to detail.
· Ability to maintain confidentiality and comply with HIPAA regulations.
· Bilingual skills are a plus, but not required.
· Commitment to the mission and values of the free medical clinic and serving vulnerable populations with dignity and respect.
· Ability to appropriately delegate tasks as needed.
· Maintain clinic protocols and policies and update accordingly.
· Assist in telehealth visits, including the use of peripherals and assessments as instructed by the medical provider.
