Job Description
Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
Deliver expert pharmaceutical care in a dynamic long-term care (LTC) and mail order environment, supporting patient health and safety through accurate medication management and interdisciplinary collaboration. This contract-to-hire opportunity offers a 13-week assignment with the potential to transition to a permanent role, working rotating first and second shifts with weekend coverage, providing variety and work-life balance.
Key Responsibilities:
- Review and interpret physician orders for accuracy, appropriateness, and safety in accordance with LTC standards.
- Dispense medications accurately within a high-volume mail order workflow.
- Maintain and update patient medication profiles to reflect current therapy.
- Monitor therapeutic responses and proactively identify, prevent, or resolve medication-related issues for LTC residents.
- Conduct medication regimen reviews to ensure compliance with safety, efficacy, and LTC regulatory requirements.
- Provide drug information and education to caregivers, nurses, and healthcare professionals.
- Collaborate with interdisciplinary teams to develop and adjust individualized care plans.
- Ensure compliance with all federal and state pharmacy laws and documentation standards.
- Manage data and workflow queues to ensure accurate order entry and mail order processing.
- Support medication management services, including adherence monitoring relevant to LTC populations.
Qualifications:
- Active Massachusetts pharmacy license required.
- Experience in retail or long-term care pharmacy settings.
- PharmD degree preferred but not required.
- Proficient in working with pharmacy order entry frameworks and data management.
- Ability to work rotating shifts (8am–4:30pm and 2:30pm–11pm) including weekends.
- Strong attention to detail and commitment to patient safety.
