Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

This pharmacist role offers a unique opportunity to work in a Long Term Care (LTC) mail order environment, focusing on accurate medication dispensing and patient safety. The position involves rotating first and second shifts (8:00 am – 4:30 pm and 2:30 pm – 11:00 pm) with weekend rotations, structured over a 13-week contract with potential for direct hire.

Key Responsibilities:

  • Review and interpret physician orders for accuracy, appropriateness, and safety.
  • Dispense medications according to prescriptions and LTC standards within a high-volume mail order setting.
  • Maintain and update patient medication profiles to reflect current therapies.
  • Monitor therapeutic responses and address drug-related problems for LTC residents.
  • Conduct medication regimen reviews ensuring safety, efficacy, and regulatory compliance.
  • Provide drug information and education to caregivers, nurses, and healthcare professionals.
  • Collaborate with interdisciplinary teams to tailor individualized care plans.
  • Comply with federal and state pharmacy laws, LTC standards, and documentation requirements.
  • Perform data management tasks including order verification and mail order workflow oversight.
  • Support medication adherence monitoring and resolve therapy issues common in LTC populations.

Desired Qualifications:

  • Pharmacy experience in retail or long term care settings.
  • PharmD degree is a plus.
  • Valid Massachusetts pharmacy license.
  • Familiarity with order entry systems such as Frameworks is beneficial.
