Summary

SUMMARY: The Palliative Care Physician will provide Palliative Care Consultation services for Alameda Health System. Services will be on-site at Highland Hospital, San Leandro Hospital and Alameda Hospital.

DUTIES & ESSENTIAL JOB FUNCTIONS: NOTE: Following are the duties performed by employees in this classification. However, employees may perform other related duties at an equivalent level. Not all duties listed are necessarily performed by each individual in the classification. 

1.  Provide Palliative Care Consultations, in collaboration with experienced and well-established interprofessional team members (physician assistant and nurse practitioners, social workers, chaplains, nurses, and outreach worker), at Alameda Health System. Location and care setting determined based on applicant experience and interest; options include:
    Inpatient Palliative Care Consultations at Highland Hospital, a Level 1 Trauma Center providing multispecialty care and hosting residencies in Emergency Medicine, General Surgery, Internal Medicine, and Oral-Maxillofacial Surgery.
    Inpatient Palliative Care Consultations at the two smaller community hospitals within Alameda Health System: Alameda Hospital and San Leandro Hospital
    Community-Based Palliative Care in person at the Highland Care Pavilion and other wellness centers, in skilled nursing facilities, and via telehealth

2.  Symptom management, serious illness communication, advance care planning, psychosocial and spiritual care support, linkage to community resources, in collaboration with the palliative care team, tailored to the needs of the diverse and vulnerable population we serve.

3.  Collaborate with other palliative care program team members to provide education to residents in Emergency Medicine, Internal Medicine, and General Surgery, equipping them with an approach to patient and family care that highlights cultural humility, psychosocial and spiritual care, and self-reflection to promote connection and resilience. The palliative care team regularly leads residents in clinical rotations and in educational sessions, including communication skills training and symptom management workshops.

4.  Participate in and/or lead quality improvement activities to improve serious illness care within the palliative care team and across Alameda Health System.

5.  Collaborate with clinicians from a range of medical and surgical specialties and subspecialties who share a commitment to serving vulnerable populations.

MINIMUM QUALIFICATIONS:

Education: Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) is required.

Minimum Experience: Completion of accredited ACGME or AOA Internal Medicine Residency and Palliative Care Fellowship

Required Licenses/Certifications: Board eligibility in Palliative Care; Board Certification (will be expected to obtain on the appropriate schedule); Compliance with Board mandated education and training to maintain State licensure and/or board certification.

Required Licenses/Certifications: Possession of a valid license to practice medicine in the State of California;Valid Drug Enforcement Agency (DEA) Number.

Pay Range: $133.96 - $144.46 / Hour 

The pay range for this position reflects the base pay scale for the role at Alameda Health System. Final compensation will be determined based on several factors, including but not limited to a candidate’s experience, education, skills, licensure and certifications, departmental equity, applicable collective bargaining agreements, and the operational needs of the organization.
