This role involves oversight and management of the day-to-day operations within the Clinical Resource Department, focusing on Case Management, Utilization Review, and Social Work. Additionally, there is a secondary emphasis on the Clinical Documentation team. Responsibilities include overseeing discharge planning, utilization management, and care coordination for high-risk patients. The position requires ongoing development and delivery of education and process improvement initiatives for the staff.

Primary Duties and Responsibilities:

  • Manage daily operations of case management and social services staff, including scheduling and performance evaluations. Assist with managing the Clinical Documentation team as needed.
  • \n
  • Regularly visit patient units to identify problems and facilitate the discharge process.
  • \n
  • Develop and deliver continuing education programs for staff, focusing on role functions and new process implementations.
  • \n
  • Provide educational programs for new hires, including case managers, trainees, social workers, and allied health professionals, relating to discharge planning and utilization review.
  • \n
  • Monitor program compliance, create dashboards to identify improvement opportunities, and implement process improvement activities.
  • \n
  • Foster continual improvement in collaboration among physicians, nurses, and clinical resource teams.
  • \n
  • Analyze program metrics using department systems knowledge.
  • \n
  • Stay updated on regulatory changes in the case management field.
  • \n
  • Identify process improvement opportunities, present recommendations, and engage in continuous improvement culture initiatives.

    Education/Experience/Training:

    \n
      \n
    • Demonstrated competency with a minimum of 3 years acute care experience within the past 5 years in a hospital setting or case management.
    • \n
    • Working knowledge of InterQual or equivalent system.
    • \n
    • Bachelor's degree in Nursing required; Master's degree preferred.
    • \n
    • Current Massachusetts Registered Nurse license.
    • \n
    • Certification in Case Management or Clinical Documentation Integrity preferred.
    • \n
    • Two years experience in process improvement; Minimum 5 years case management experience in an acute setting.
    • \n
    • Experience in developing and presenting professional continuing education programs.
    • \n
    • Knowledge of CMS regulations.
    • \n
    • Excellent computer skills; familiarity with Windows-based software programs.
    • \n
    • Strong organizational, analytical, writing, and interpersonal skills.
    • \n
    • Management/supervisory experience preferred.
