Job Description
LanceSoft is seeking a travel nurse RN Hospice Palliative Care RN for a travel nursing job in Albany, Oregon.

- 40 hours per week
- Shift: 8 hours, days, evenings, nights, flexible, rotating

Schedule: Variable Shifts, Rotating Weekends Required Qualifications
✅ Active Oregon RN License
✅ BLS Certification
✅ Valid Driver's License
✅ Personal Vehicle & Auto Insurance
✅ Minimum 1 year RN experience
- Hospice RN experience
- Home Health experience
- Case Management experience
- BSN
- Strong end-of-life and palliative care background
- Conduct hospice nursing visits in patients' homes, assisted living facilities, SNFs, and adult foster homes.
- Perform comprehensive patient assessments and ongoing care evaluations.
- Manage pain and symptom control for terminally ill patients.
- Develop and update individualized hospice care plans.
- Educate patients and families regarding disease progression and end-of-life care.
- Coordinate care with physicians, social workers, chaplains, hospice aides, and other interdisciplinary team members.
- Provide emotional support and counseling to patients and families.
- Complete documentation in EPIC.
- Participate in weekend rotation and on-call coverage as assigned.
