Requirements:
- Board Certified/Board Eligible GYN with expertise in Minimally Invasive Gynecologic Surgery, fellowship trained, or extensive experience preferred - [REQUIRED]
- CO license (or willing to obtain) - [REQUIRED]
- Clean malpractice - [HIGHLY PREFERRED]
- Most recent NPDB Self Query Report (within 30 days active) - [REQUIRED] * no exemption, required at presentation
- Does the provider require visa support - J1 or H1B? (note in "Additional Submission Details" field) - [REQUIRED]
- DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) - [REQUIRED] * no exemptions
- Colorado License
