Travel RN Case Manager – Utilization Review & Discharge Planning

Location: Portales, NM
Start Date: 07/13/2026
Contract: 13 Weeks
Schedule: Monday–Friday, 8:00 AM – 4:30 PM
EMR: Meditech

Requirements:

Active RN License
Minimum 5 years of Acute Care Case Management experience
Strong Utilization Review and Discharge Planning experience
Experience with InterQual criteria
Swing Bed/Skilled Care coordination experience preferred
Knowledge of Medicare, Medicaid, HMO, and commercial insurance authorization processes

Responsibilities:

Utilization Review and case management
Discharge planning and care coordination
Swing Bed admissions and placement coordination
Prior authorizations for post-acute services
Daily interdisciplinary rounds and physician collaboration
MDS documentation and insurance review
