Job Summary


We are seeking a licensed Occupational Therapist to complete environmental assessments for individuals receiving community-based services. The Occupational Therapist will evaluate the individuals home and daily living environment to identify safety concerns, accessibility barriers, adaptive equipment needs, and recommendations that support independence, mobility, and quality of life.

The ideal candidate is detail-oriented, professional, compassionate, and experienced in assessing functional needs within the home or community setting. This role requires strong clinical judgment, clear documentation, and the ability to make practical recommendations that help individuals safely remain in their homes and participate in daily activities.

Responsibilities


  • Complete in-home environmental assessments for individuals with developmental disabilities, physical disabilities, mobility limitations, or functional support needs.
  • Assess the individuals ability to safely access and use key areas of the home, including entrances, bathrooms, bedrooms, kitchens, living areas, and pathways.
  • Identify environmental barriers that may impact safety, mobility, independence, or participation in activities of daily living.
  • Evaluate the need for home modifications, adaptive equipment, durable medical equipment, assistive technology, or safety supports.
  • Provide written recommendations for modifications such as grab bars, ramps, widened doorways, bathroom adaptations, transfer supports, flooring changes, and other accessibility improvements.
  • Review the individuals functional abilities, mobility needs, caregiver support needs, and risk factors during the assessment process.
  • Prepare clear, timely, and professional assessment reports that include findings, recommendations, and clinical justification.
  • Collaborate with care teams, support coordinators, families, caregivers, providers, and other professionals as needed.
  • Ensure assessments and documentation meet applicable agency, state, waiver, and program requirements.
  • Maintain confidentiality and comply with HIPAA and all applicable professional standards.
  • Communicate findings in a respectful, person-centered manner that supports the individuals goals, preferences, safety, and independence.
Qualifications


  • Current Occupational Therapist license in the state of Maryland.
  • Experience completing home assessments, environmental assessments, accessibility evaluations, or working with individuals with disabilities preferred.
  • Strong knowledge of adaptive equipment, home modifications, mobility supports, and activities of daily living.
  • Excellent written and verbal communication skills.
  • Ability to work independently and manage documentation deadlines.
  • Reliable transportation for home-based assessments.
  • Compassionate, professional, and person-centered approach to care.
