Responsibilities
- Perform initial PT evaluations and develop individualized plans of care for residents in a LTC/home health environment.
- Deliver therapeutic interventions including therapeutic exercise, gait and balance training, range of motion and strength activities, functional mobility, and pain management as appropriate.
- Monitor patient progress, adjust treatment plans, document outcomes, and communicate findings to the interdisciplinary care team.
- Educate residents and caregivers on exercises, safety, and activities of daily living (ADLs) to promote independence.
- Ensure patient safety during therapy sessions and maintain accurate, compliant documentation in the medical record.

Requirements
- Active Physical Therapist license in the United States (or eligibility per standard state requirements)
- Ability to work effectively in a long-term care setting
- Strong assessment, treatment planning, and documentation skills
- Excellent written and verbal communication; solid teamwork and collaboration with an interdisciplinary care team
- Commitment to clinical quality, patient safety, and professional standards
