Job Description

Position Overview
The Occupational Therapist will evaluate patients' functional abilities and create individualized treatment plans tailored to their unique goals and needs. Working closely with an interdisciplinary team, this position focuses on improving patients' ability to perform everyday activities, supporting recovery, and facilitating successful transitions of care.
Key Responsibilities
• Conduct comprehensive evaluations to assess patients' functional limitations and rehabilitation needs.
• Develop and implement personalized treatment plans designed to maximize independence and quality of life.
• Provide therapeutic interventions that address physical, cognitive, and functional challenges.
• Monitor patient progress and modify treatment approaches as needed to achieve optimal outcomes.
• Educate patients and family members on therapeutic techniques, adaptive strategies, and equipment use.
• Collaborate with physicians, nurses, case managers, and other healthcare professionals to coordinate care.
• Maintain accurate and timely documentation of evaluations, treatments, and patient progress.
• Contribute to discharge planning and continuity of care efforts.
Qualifications
• Licensed or eligible for licensure as an Occupational Therapist.
• Previous inpatient rehabilitation experience preferred.
• Strong interpersonal and communication skills.
• Team-oriented mindset with the ability to work independently when appropriate.
• Organized, self-motivated, and dedicated to delivering high-quality patient care.
