Job Title: Licensed Practical Nurse (LPN) – Long Term Care (LTC)

Location: Wayland, New York
Duration: 13–52 Weeks

Job Summary

We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to provide nursing care in a long-term care environment serving individuals with developmental and intellectual disabilities. Working under the supervision of a Registered Nurse, the LPN will assist with medication administration, treatments, patient monitoring, and activities of daily living while promoting resident safety, comfort, and well-being.

Key Responsibilities
  • Administer medications, injections, and prescribed treatments.
  • Perform tube feedings and other nursing procedures within scope of practice.
  • Collect specimens and assist with diagnostic testing as required.
  • Provide colostomy care, wound care, and dressing changes.
  • Monitor resident conditions and report significant changes to the healthcare team.
  • Obtain and document vital signs and other clinical observations.
  • Assist residents with activities of daily living, including bathing, feeding, mobility, and personal care.
  • Maintain accurate documentation and resident records.
  • Support a safe, clean, and therapeutic living environment.
  • Collaborate with nurses, physicians, and interdisciplinary team members to coordinate resident care.
  • Float to other assigned locations as needed based on operational requirements.
Requirements

Licensure

  • Active New York State Licensed Practical Nurse (LPN) License required.

Experience

  • Long Term Care (LTC) experience preferred.
  • Experience caring for individuals with developmental or intellectual disabilities preferred.
  • Strong clinical and resident care skills required.
Skills & Qualifications
  • Medication administration and treatment management.
  • Tube feeding and colostomy care.
  • Vital signs monitoring and patient assessment.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Strong documentation and organizational abilities.
