Responsibilities

• Provide skilled physical therapy services in patients' homes according to established care plans
• Evaluate patient progress and adjust treatment plans as necessary
• Communicate effectively with patients, families, and healthcare team members
• Maintain accurate documentation and comply with company policies and regulatory requirements
• Demonstrate professionalism and empathy while fostering patient independence

Requirements

• Active Physical Therapist license in the state of Michigan
• Proven experience in homecare physical therapy
• Excellent interpersonal and communication skills
• Strong organizational skills and attention to detail
• Reliable transportation and ability to travel to patient homes in the Southeast MI Region.
