Home Health Aide Needed - Full-time / Part-time

I need an adult care provider to assist in transportation after an outpatient procedure. The procedure will occur in the afternoon on July 07 with the exact time confirmed later. I am ambulatory and the procedure will done under local anesthesia, therefore I do not need medical assistance/wheelchair. The duties of this job include picking me up from the surgical center and transporting me back home approximately 5 miles away. No medical assistance is required but companion may need to sign release form from surgical center.
