General Purpose


Under the leadership of the direct supervisor, plan,implement, and evaluate health education activities inthe Bear River Health District.


Minimum Qualifications (Education & Experience)


Health Educator I: Graduation from an accredited college or university with a B.S. degree in Health Science, Public Health, Community Health, or other closely related field AND 1 year of full time paid professional employment in community or public health; PLUS CHES eligible.


Health Educator II: Graduation from an accredited college or university with a B.S. degree in Health Science, Public Health, Community Health, or other closely related field; AND 2 years of full-time paid professional employment in community or public health; PLUS CHES eligible.


Substitutions on a year-for-year basis as follows: Additionalgraduate study in a public or community health program for therequired full-time paid professional employment requirement.


Minimum Qualifications (Necessary Knowledge, Skills &Abilities)


  • Knowledge of public health education models, behavior changetheories, and primary prevention techniques.
  • Knowledge of healtheducation methods and preventative health practices.
  • Knowledge ofcommunity analysis and survey techniques, needs assessment, andprinciples of community organization.
  • Knowledge of educational toolsand use of the media for information dissemination.
  • Knowledge ofrisk/protective factor models.
  • Knowledge of Social Determinants ofHealth.
  • Skills to lead discussions and teach large or small groups.
  • Skills in computer programs including Google Workspace, wordprocessing, spreadsheets, databases, desktop publishing, internet,and email use.
  • Skilled in written, verbal, and public communication.
  • Ability to establish and maintain effective working relationships withcoworkers, other agencies, and community leaders.
  • Ability to workwell under pressure, manage multiple tasks simultaneously, work wellin team situations, meet deadlines, and provide positive customerservice.
  • Ability to proficiently use computers.
  • Ability to write technicalreports, develop curriculum, and make effective presentations.
  • Abilityto be an active member of a team.
  • Ability to be self-motivated and self-directed.


Special Qualifications


  • Must possess or be able to obtain a valid Driver License at the time of employment.
  • Must be able toprovide own transportation to assigned office.
  • Must be able to lift 50pounds and be able to perform strenuous activitiesrequiring flexibility, stamina, and overall general goodhealth.
  • Must have a flexible schedule and be availablefor work in the late evening, early morning, andweekends as job requires.
  • Must pass a criminalbackground check and drug screening test prior toemployment.


Summary of Duties


  • Tobacco CDC Program Manager - Lead development of programgoals/objectives, coordinate staff objectives with specific allocatedtime, monitor contract expenses, assure completion of work plan,assure completion of reports.
  • Diabetes Program Manager - Leaddevelopment of program goals/objectives, coordinate staff objectiveswith specific allocated time, monitor contract expenses, assurecompletion of work plan, assure completion of reports.
  • Cardiovascular Health Program Manager - Lead development ofprogram goals/objectives, coordinate staff objectives with specificallocated time, monitor contract expenses, assure completion of workplan, assure completion of reports.
  • Collaborate with communitypartners to address diabetes prevention & hypertension and link toservices available.
  • Plan, implement, and evaluate tobacco preventionand control policies, strategies, and cessation services.
  • Mobilizepartnerships to educate and implement tobacco policies at worksites,community based organizations, multi-unit housing properties,healthcare providers, and other needed areas of thecommunity.
  • Assist with linking individuals to tobacco cessationservices.
  • Inform and educate diverse audiences and populations ondiabetes, cardiovascular health, and tobacco preventioneducation.
  • Assist with coordinating the Governing Youth Council(GYC) coalition in the Bear River health district.
  • Attend virtualmeetings, including monthly department updates, state workgroups,and local coalition/committees.
  • Enhance skills and knowledge throughprofessional development opportunities.
  • Complete quarterly contractreports.
  • Serve as a resource for public health requests.
  • Respond topublic health emergencies as required by the Department.
  • Completesother duties as assigned.
