RN Referral Coordinator & Healthcare Navigator
Department: Clinical Services
Reports To: Clinic Manager
Position Summary
The Medical Referral Coordinator & Healthcare Navigator provides dual support within the clinic by managing routine referrals and delivering advanced care coordination for patients with complex medical and social needs. This role ensures timely access to specialty care while addressing barriers through coordination with community resources and healthcare partners.
Responsibilities
Referral Coordination (Routine Patients)
Send, process and track referrals for specialty care.
Ensure completeness of required documentation prior to submission.
Coordinate with volunteer specialists and partner organizations.
Communicate with patients regarding expectations/instructions & follow up throughout referral process
Monitor referral completion, obtain records, and document outcomes in EMR.
Escalate patients with barriers for Healthcare Navigation
Healthcare Navigation (Complex Patients)
Identify patients with medical, social, or financial barriers.
Perform needs assessments and develop care coordination plans.
Coordinate multi-specialty and community-based services.
Address barriers including transportation, medication access, and social needs.
Provide ongoing follow-up and serve as primary contact for complex patients.
Advocate for timely access to necessary care.
Escalate to primary care provider when necessary
Community Resource Coordination
Maintain knowledge of local healthcare and social service resources.
Establish and maintain community partnerships.
Connect patients to support services and assistance programs.
Administrative / Quality
Maintain accurate EMR & Excel Spreadsheet documentation for referral tracking.
Assist with reporting and quality improvement initiatives.
Support clinic workflow and team communication.
Cross train on clinical skills to provide backup when needed
Qualifications
Registered Nurse (RN) required
Minimum 2 years' experience in care coordination, referrals, or case management
Proficient in Excel Spreadsheets and EMR navigation

What it does
Provides primary care and specialty medical services.
Offers medications for acute and chronic conditions (excluding narcotics).
Provides laboratory testing, X-rays, and referrals for specialty care.
Offers women's health services such as Pap smears and mammograms.
Operates diabetic care, nutrition counseling, and patient education programs.
Provides pediatric sick visits and school physicals.
Offers behavioral health services through referrals from clinic practitioners.
Coordinates additional services such as physical therapy, ophthalmology, ENT care, and wellness programs.
