We are currently seeking an experienced and motivated Cardiology Nurse Practitioner to join our growing team in Virginia. This is an excellent opportunity for a provider who thrives in a collaborative environment and is passionate about cardiovascular patient care across multiple care settings, including outpatient offices, surgical centers, and long-term care facilities.
Position Summary
The Cardiology Nurse Practitioner will work closely with cardiologists, vascular surgeons, and clinical staff to evaluate, diagnose, and manage patients with cardiovascular and peripheral vascular conditions. The NP will play a key role in patient education, follow-up care, medication management, and care coordination within a multidisciplinary setting.
In addition to office-based care, the NP will provide consultative cardiology services and perform routine rounding at skilled nursing facilities, rehabilitation centers, assisted living facilities, and nursing homes throughout the Virginia territory. The provider will serve as an extension of the cardiology team, ensuring continuity of care and timely management of cardiovascular conditions in post-acute and long-term care settings.
This role requires regular travel between outpatient offices, surgical centers, and nursing facilities throughout Virginia and surrounding service areas.
Key Responsibilities
Clinical Responsibilities
· Perform comprehensive cardiovascular assessments and physical examinations
· Evaluate and manage patients with hypertension, coronary artery disease, peripheral arterial disease (PAD), venous disease, heart failure, arrhythmias, and related cardiovascular conditions
· Order, review, and interpret diagnostic testing including EKGs, vascular studies, laboratory work, echocardiograms, and imaging results
· Develop and implement treatment plans in collaboration with supervising physicians
· Manage medications and provide patient counseling regarding cardiovascular health and disease prevention
· Provide pre-operative and post-operative evaluations for vascular and cardiovascular procedures
· Document patient encounters accurately and timely within the EMR system
· Participate in care coordination and follow-up management for high-risk patients
· Collaborate with medical assistants, nurses, ultrasound technologists, and administrative staff to ensure efficient patient flow
Nursing Home & Long-Term Care Rounding
· Conduct routine cardiology rounds at skilled nursing facilities, rehabilitation centers, assisted living communities, and nursing homes throughout Virginia
· Perform cardiovascular consultations for newly admitted and established patients in long-term care settings
· Evaluate and manage acute and chronic cardiovascular conditions, including heart failure exacerbations, arrhythmias, hypertension, edema, PAD, and anticoagulation management
· Collaborate with facility physicians, nurses, case managers, therapists, and caregivers to coordinate patient care plans
· Review hospital discharge summaries and ensure continuity of cardiovascular care following hospitalizations
· Develop treatment recommendations and communicate findings to supervising cardiologists and referring providers
· Identify patients requiring escalation of care or referral to outpatient clinics, imaging services, or procedural intervention
· Maintain timely documentation and billing compliance for all facility visits and consultations
Patient Care & Education
· Educate patients and families on diagnoses, medications, lifestyle modifications, and treatment plans
· Promote preventative cardiovascular care and PAD awareness initiatives
· Ensure high-quality patient experience and compassionate care delivery
· Assist in improving clinical workflows and patient outcomes
· Provide education to nursing facility staff regarding cardiovascular disease management and treatment protocols
Operational Responsibilities
· Maintain compliance with clinical protocols, HIPAA regulations, and practice policies
· Support quality improvement initiatives and performance goals
· Participate in provider meetings, case reviews, and interdisciplinary collaboration
· Assist with outreach and continuity of care efforts when needed
· Maintain productive relationships with nursing home administrators, directors of nursing, and referral sources throughout the territory
Qualifications
· Current Nurse Practitioner license in the Commonwealth of Virginia
· Board Certification as an Adult-Gerontology NP, Family NP, or Acute Care NP
· DEA license and prescriptive authority
· Minimum of 2 years of cardiology, vascular, internal medicine, geriatric medicine, or critical care experience preferred
· Strong knowledge of cardiovascular disease management and outpatient clinical workflows
· Experience providing care in skilled nursing facilities, rehabilitation centers, or long-term care settings preferred
· Experience with EMR systems (Athena experience preferred)
· Excellent communication, clinical judgment, and organizational skills
· Ability to work independently while functioning effectively within a collaborative team environment
· BLS Certification required
· ACLS Certification preferred
· Valid driver’s license and reliable transportation for travel throughout Virginia
Preferred Experience
· Outpatient cardiology or vascular surgery experience
· Skilled nursing facility (SNF) or long-term care rounding experience
· Peripheral arterial disease (PAD) management
· Heart failure management and transitional care services
· Stress testing, cardiac monitoring, and vascular diagnostics familiarity
· Experience working in high-volume specialty practices
· Experience coordinating care across multiple clinical sites
