Hospice Social Worker

We are seeking a full-time Hospice and part-time Social Worker who would like to join our hospice team!

Job Description:

• Hospice Social Worker, M-F, Dayshift
• Create your own visit schedule and have work/life balance.
• As a Hospice Social Worker, you will provide patients and families education and support throughout the end-of-life process and address patient's physical, emotional and spiritual needs.
• You will work closely with the hospice interdisciplinary team to create and implement a holistic plan of care congruent with the patient's and family's wishes, focusing on comfort, support and quality of life.
