Physical Therapist

Physical Therapist (PT) – Home Health

Location: Hudson County, NJ
Schedule: Monday–Friday, 8:00 AM–5:00 PM, plus one weekend per month
Mileage Reimbursement: $0.43 per mile

We are seeking a compassionate and motivated Physical Therapist (PT) to join our Home Health team. In this role, you will provide skilled therapy services to patients in their homes, helping them improve mobility, independence, and overall quality of life.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans
• Review physician orders, diagnoses, medical history, precautions, and contraindications
• Establish measurable goals and provide evidence-based therapeutic interventions
• Monitor patient progress and modify treatment plans as needed
• Communicate effectively with physicians, nurses, caregivers, and interdisciplinary team members
• Provide patient and family education regarding treatment plans and safety
• Recommend and train patients on adaptive equipment and assistive devices
• Complete all clinical documentation accurately and within 24 hours of patient visits
• Participate in weekly case conferences and interdisciplinary meetings
• Supervise Physical Therapist Assistants (PTAs) and therapy aides in accordance with state regulations
• Maintain compliance with HIPAA, company policies, and professional practice standards

Productivity Expectations

• Minimum benchmark of 30 points per week
• Productivity expectations may increase based on patient census and agency needs
• Weekly case conference every Wednesday at 8:45 AM (approximately 1.5 hours)

Requirements

• Active New Jersey Physical Therapy License
• Valid driver's license, reliable transportation, and auto insurance
• Strong clinical assessment and treatment skills
• Ability to work independently while managing a flexible patient schedule
• Excellent communication and organizational skills

Preferred Qualifications

• Home Health experience
• HomeCare HomeBase (HCHB) experience
• PointCare and/or PointClickCare experience
• Proficiency with Microsoft Teams and healthcare technology platforms
• Experience mentoring or training new clinicians
