Job Description
Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
This opportunity is seeking a skilled Long Term Care Pharmacist to support medication management for skilled nursing facilities, personal care homes, and hospice care settings. The role offers a contract assignment with an initial 1-month timeline and potential for extension, providing 40 hours per week within an established pharmacy schedule.
Work Hours:
- Monday through Friday: 8:00 AM – 6:00 PM
- Saturday: 8:00 AM – 4:00 PM
- Sunday: 8:00 AM – 3:00 PM
Key Responsibilities:
- Review and verify medication orders entered by data entry technicians for various long-term care facilities.
- Monitor and check blister-packed medications as well as sterile and non-sterile preparations.
- Respond to clinical inquiries from nursing staff, hospice nurses, and physicians via telephone.
- Conduct controlled substance counts and maintain accurate perpetual inventory records.
- Inspect and verify emergency medication boxes to ensure readiness and compliance.
Required Qualifications:
- Bachelor's degree in Pharmacy.
- Active pharmacist license in good standing with the state of Pennsylvania.
- At least 1 year of pharmacist experience, preferably with long-term care exposure.
- Proficient in pharmacy operating systems including QS1, Docutrack, and PCC.
- Physical ability to sit, stand, perform tasks requiring fine motor skills and visual acuity, and lift up to 30 pounds.
