Long Term Care Staff Pharmacist in Valencia, PA

Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

A Long Term Care Pharmacist position is available for a dedicated professional with experience in skilled nursing, personal care, and hospice pharmacy services. This role requires attention to detail and excellent communication skills to support healthcare providers in managing medication needs effectively.

Location: Valencia, PA
Job Type: Contract (1-month ongoing with possible extension)
Schedule: 40 hours per week

• Monday through Friday: 8:00 AM – 6:00 PM
• Saturday: 8:00 AM – 4:00 PM
• Sunday: 8:00 AM – 3:00 PM

Key Responsibilities:

• Review and verify medication orders entered by data entry technicians for various facilities including skilled nursing, personal care, and hospice.
• Inspect packaged medications, including blister-packed, sterile, and non-sterile medications for accuracy.
• Field phone inquiries from nursing facilities, hospice nurses, and physicians, providing clinical guidance as needed.
• Conduct controlled substance counts and maintain accurate documentation in the perpetual inventory.
• Check and manage emergency medication boxes to ensure availability and compliance.

Required Qualifications:

• Bachelor’s degree in Pharmacy.
• Active pharmacist license in good standing with the state of Pennsylvania.
• Minimum of 1 year of pharmacy experience; long term care experience preferred.
• Physical ability to sit, stand, perform fine motor tasks, maintain visual acuity, and lift up to 30 pounds.

Technology Experience:

• Proficient with pharmacy operating systems including QS1, Docutrack, and PCC.
