Job Details
- American Heart Association BLS certification required for all direct patient care personnel
- ACLS certification required for RNs in CCU, ED, PACU, MICU, SICU (included here as PCU is a critical care step-down unit)
- State nursing license verification must be dated within 30 days of start
- Specialty specific competency testing must be completed within a year
- Trinity Code of Conduct Form signed and on file
- Trinity I9 completed by agency
- Confidentiality and security agreement signed and on file
- Integrity and audit services form signed prior to start
- Trinity Conflict of Interest Disclosure Form signed and on file
- Criminal background check including federal, nationwide, county from last 7 years, SSN verification, dated within 30 days of start
- Education verification of highest academic degree or diploma
- Exclusion lists (EPLS, OIG) checked and dated within past 30 days
- Joint Commission CORE exam within a year
- NSO report dated within 30 days including aliases and statewide check for MI
- System access form completed and uploaded one week before start
- Fit test required
- Trinity 10 panel drug screen completed within 30 days of start
- Proof of COVID-19 vaccine or approved declination/exemption
- Proof of MMR vaccination or IgG titer
- Hepatitis B vaccination series started prior to start
- Influenza vaccine proof or declination (optional)
- 2 Step PPD or IGRA test with chest xray if positive
- Varicella vaccination proof or IgG titer
- Physical/health statement within last year
- Tdap vaccine proof or declination if no pediatric interaction
- Trinity Respirator Evaluation with physician signature
- Trinity Skills Checklist provided by agency for clinical contractors
Job Requirements
- Shift: 7:00 PM to 7:30 AM (12.5 hour night shift)
- Schedule includes working every other or every third holiday
