RN – Home Health Case Manager
Location: Vermont & New Hampshire (Bradford, Randolph, Springfield, White River Junction, VT & Lebanon, NH)
13-Week Contract
Schedule: Days
Holiday Coverage: Not Required
Requirements:
• Active Vermont and New Hampshire RN License or Compact RN License
• BLS required
• Minimum 2 years of Home Health experience
• Graduate of an accredited nursing program
• Experience with EPIC EMR preferred
• OASIS documentation experience preferred
• Strong assessment, care coordination, and patient education skills
Preferred Qualifications:
• Previous RN Case Management experience
• Ability to work independently in the field
• Excellent communication and interdisciplinary coordination skills
