We are looking for a per diem Certified Occupational Therapist Assistant (COTA) with recent Home Health experience to visit patients in their homes in the Riverside and Moreno Valley area.

Responsibilities and Duties:

• Assess patient's condition under a OT during every visit and chart observations
• Perform evaluation tasks, including vital signs
• Follow prescribed treatments, following plan of care and assess progress of healing
• Provide appropriate timely feedback to Occupational Therapist and other professionals
• Charting and documenting on EMR system.
