Mobile Medical is seeking a compassionate Licensed Clinical Social Worker (LCSW) to provide behavioral health support, counseling, care coordination, and resource management to patients throughout Duval County.

Position Summary

As an LCSW, you will provide counseling, psychosocial assessments, care coordination, and community resource support to patients receiving care in their homes and through telehealth services.

You'll collaborate closely with providers, caregivers, and interdisciplinary healthcare teams to improve patient outcomes and overall well-being.

Key Responsibilities

• Conduct psychosocial assessments and behavioral health evaluations
• Provide individual counseling and supportive therapy
• Develop and implement treatment plans
• Identify barriers to care and connect patients with community resources
• Assist patients with social, emotional, financial, and behavioral health needs
• Coordinate care with providers and interdisciplinary teams
• Educate patients and families regarding available support services
• Maintain accurate and timely documentation
• Ensure compliance with Medicare and company documentation standards
• Support continuity of care and positive patient outcomes

Qualifications

Required

• Master's Degree in Social Work (MSW)
• Active Florida Licensed Clinical Social Worker (LCSW) License
• Minimum of 1 year of clinical counseling experience
• Strong communication and relationship-building skills
• Valid Florida Driver's License and reliable transportation

Preferred

• Medical Social Work experience
• Home Health experience
• Telehealth experience
• Experience working with adult and geriatric populations
• Knowledge of community resources and care coordination
