Position Type: Full-Time / Part-Time (Flexible Scheduling Available)



About the Role


We are seeking a compassionate, detail-oriented Physician Assistant (PA-C) to support our growing medical team in skilled nursing facilities and personal care homes. This role focuses on providing high-quality, patient-centered care to older adults and medically complex patients in long-term care settings.

Key Responsibilities



  • Conduct comprehensive medical assessments for nursing home and personal care home residents
  • Perform routine follow-ups, acute visits, admissions, and discharge planning
  • Manage chronic conditions and address acute medical issues in collaboration with supervising physicians
  • Order, interpret, and follow up on diagnostic tests and labs
  • Document all patient encounters clearly and accurately in the electronic medical record
  • Communicate effectively with nursing staff, facility leadership, families, and interdisciplinary teams
  • Participate in care plan meetings and contribute to quality improvement initiatives
  • Ensure compliance with facility policies, state regulations, and clinical best practices






Qualifications



  • Current Physician Assistant license in the state of practice
  • National certification (NCCPA)
  • Experience in geriatrics, internal medicine, long-term care, or post-acute care preferred (new grads with strong interest encouraged to apply)
  • Strong clinical judgment and time-management skills
  • Excellent communication and interpersonal abilities
  • Ability to work independently while collaborating within a multidisciplinary team
