Job Description
We’re looking for a motivated Memory Care Coordinator to join our team!
This position assists the Resident Services Director and/or the Health Services Director with monitoring resident health, including:
- Record Keeping
- Scheduling of Personal Care Assistants, developing daily tasks and making assignments
- Self- Medication Monitoring and Record Keeping
- Initiate and Maintain Emergency Packets
- Organization of Resident Charts
Job Responsibilities:
- Medication management: ensure that medications are properly stored and cared for.
- Observe residents' health status, take actions to address concerns with scope of practice, record changes, and notify Resident Services Director.
- Lift/transfer residents as needed
- Maintain residents’ records daily in a timely manner and in accordance with company policy and procedures (includes documentation, med distribution, leisure activities, incidents and observations, charting any changes in physical conditions or behavior.
- Schedule Personal Care Assistants and develop daily tasks and assignments
- Evaluate resident care needs and determine the required level of care.
Requirements:
- High school diploma or equivalent certificate
- Valid First Aid and CPR Certification required.
- Must possess a certified nursing assistant certification.
- Must have reliable source of transportation
- Must have a team-player mindset with attention to details
- Timely and organized
- Able to speak, read, write and understand the English language fluently.
- Able to pass a background check and drug screen
Work Schedule:
- Tuesday – Saturday (schedule may vary)
