Job Description
In this role, you'll oversee the daily operations of the trauma case management program, ensuring effective care coordination, appropriate resource utilization, and smooth transitions throughout the patient journey. You'll collaborate closely with physicians, nurses, social workers, and hospital leadership to improve patient outcomes and drive operational excellence.
What You'll Be Doing
• Provide leadership and oversight for the trauma case management team
• Manage utilization review, discharge planning, and care coordination activities for trauma patients
• Partner with interdisciplinary teams to support efficient patient flow and timely transitions of care
• Ensure compliance with regulatory requirements, payer guidelines, and hospital policies
• Support quality improvement initiatives focused on patient outcomes, length of stay, and resource management
• Coach, mentor, and develop team members while fostering a collaborative work environment
• Assist with complex case reviews and discharge planning challenges
• Monitor department performance and identify opportunities to improve processes and patient care
What You'll Need
• Active Texas RN license or LMSW required
• LCSW preferred
• Case Management Certification required
• Five or more years of experience in case management, utilization management, discharge planning, or related healthcare programs
• At least three years of hospital-based nursing or social work experience
• Three or more years of leadership or supervisory experience
• Strong understanding of acute care operations, care coordination, and utilization management practices
