M - 9:00a - 5:00p
T, W - 7:00a - 3:00p
TH – 10:00a – 6:00p
F - 6:00a - 2:00p
Family Practice, Occ Health, minor procedures, health coaching.
DOT certified
Clean, Active OH license
DEA
2 years or more of NP work experience.
Malpractice, Accommodations provided. Mileage reimburses.
