Licensed Practical Nurse (LPN) – Sub-Acute Rehabilitation

Location: Newark, NJ
Schedule: Per Diem | Flexible Scheduling
Shifts Available: Days, Evenings, Nights, and Double Shifts
Start Date: ASAP

Position Overview

We are currently seeking Licensed Practical Nurses (LPNs) for a Sub-Acute Rehabilitation and Long-Term Care facility in Newark, New Jersey. This opportunity offers flexible scheduling, competitive compensation, and the ability to work the shifts that best fit your availability.

The ideal candidate will have experience in a skilled nursing, rehabilitation, or long-term care setting and be committed to providing high-quality resident care.

Must Have
  • Active New Jersey Licensed Practical Nurse (LPN) License
  • Current American Heart Association (AHA) BLS Certification
  • Minimum of 6 months recent Long-Term Care, Skilled Nursing, or Sub-Acute Rehabilitation experience
  • Strong clinical assessment and documentation skills
  • Ability to work independently and as part of a multidisciplinary team
  • Excellent communication and resident care skills
Nice to Have
  • Experience in a Sub-Acute Rehabilitation setting
  • Experience with Electronic Medical Records (EMR)
  • Experience with admissions, discharges, and rehabilitation patients
  • IV therapy experience
  • Flexible availability for multiple shifts, weekends, or doubles
Responsibilities
  • Administer medications and treatments as prescribed
  • Monitor, assess, and document resident conditions
  • Report changes in resident status to the supervising nurse and physician as appropriate
  • Maintain accurate and timely clinical documentation
  • Assist with admissions, transfers, and discharges
  • Collaborate with physicians, nurses, therapists, and other healthcare professionals
  • Ensure compliance with facility policies, infection control standards, and regulatory requirements
  • Provide compassionate, resident-centered nursing care
