Position Title: MDS Nurse

Position Type: Part-Time, Non-Exempt

Shift: (7:am-3:30pm) Tuesday, Wednesday, and Thursday

Qualifications:

  • Current Washington RN or LPN licensure.
  • 2+ years Acute or Long Term Care Experience.
  • Previous MDS Nurse experience preferred.

Essential Duties and Responsibilities:

  • Responsible for the coordination of all MDS assessments.
  • Ensures the accuracy, completeness, and timeliness of the Minimum Data Set (MDS) for all Residents of the Health Care Center.
  • Assists the Primary Care Nurses (PCNs) in the completion of the quarterly and annual Resident Revues and facilitates the timeliness of these reports in reaching the medical record.
  • Assists with determination of Resident eligibility for Medicare coverage and facilitates compliance with Medicare requirements for documentation and continued Resident certification.
  • Facilitates coordination of care between the disciplines of Nursing, Rehabilitation Services, Social Services, and other disciplines as necessary to ensure that Residents are receiving coordinated, comprehensive, and continuous care as determined in care conferences and clinicals.
  • In cooperation with Social Services facilitates the admission of new residents to ensure a smooth transition into the Care Center. This includes monitoring the admission process to ensure the completeness and accuracy of all admission documents.
  • Complete RAI process on all assigned residents according to OBRA and PPS guidelines on a timely basis.
  • Attend Medicare Meetings.
  • Ensure consultant’s recommendations are implemented.
  • Review Quality Indicator reports and assist with corrections as needed.
  • Work with MRC to schedule RAIs for assigned residents.

Position Wage:

Wage decided upon employment, depending on licensure and previous experience.

LPN: $41.29 - $49.11 hourly

RN: $49.67 - $59.30 hourly

Upon acceptance of an offer, candidates must be able to pass a drug screen, reference checks, and background check prior to starting work with eliseo. Upon first day of hire, all staff are required to provide documentation of influenza vaccination (Seasonally for Influenza). Additionally, all staff will undergo tuberculosis testing upon their first day and annually.
