Job Description
POSITION SUMMARY
The Certified Nursing Assistant (CNA) plays a vital role in supporting individuals enrolled in the
Recuperative Care (RC) and Short-Term Post-Hospital Housing (STPHH) programs. The CNA provides
non-clinical and supportive healthcare services to individuals transitioning from hospitalization who
may have complex medical, behavioral health, and social service needs. Working under the direction of
licensed nursing staff and in collaboration with interdisciplinary care teams, the CNA assists clients in
achieving medical stability, improving activities of daily living, and promoting successful recovery
within a community-based setting.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The essential functions include, but are not limited to the following:
● Provide compassionate, trauma-informed, and person-centered care to clients
recovering from acute medical conditions.
● Assist clients with activities of daily living (ADLs), including personal hygiene, grooming,
dressing, mobility, and meal support.
● Monitor and report changes in clients' physical condition, functional status, or behaviors
to licensed nursing staff.
● Assist with basic health-related services, including vital signs, wound care support,
medication observation (as directed), and health monitoring within scope of practice.
● Support clients in attending medical appointments and following discharge
recommendations.
● Reinforce health education provided by licensed medical staff, including chronic disease
management, wound care, medication adherence, and healthy lifestyle practices.
● Collaborate with nurses, case managers, behavioral health staff, and community
providers to support individualized care plans.
● Assist clients with mobility and ambulation, including the safe use of assistive devices
such as walkers, wheelchairs, and crutches.
● Maintain accurate, timely, and complete documentation of services provided, client
interactions, and observed concerns in accordance with program standards.
● Support infection prevention and control practices, including maintaining a clean and
safe environment.
● Respond appropriately to client crises, emergencies, or medical concerns and follow
established program protocols.
● Participate in interdisciplinary team meetings, case conferences, and care coordination
activities.
● Maintain client confidentiality and adhere to all HIPAA, program, and regulatory
requirements.
MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)
● Current and valid Certified Nursing Assistant (CNA) certification in the State of California.
● Current CPR and First Aid certification preferred.
● Experience working with individuals recovering from hospitalization, chronic medical
conditions, homelessness, behavioral health conditions, substance use disorders, or other
vulnerable populations preferred.
● Knowledge of activities of daily living (ADL) support, infection control practices, and basic
healthcare procedures.
● Ability to communicate effectively with clients, healthcare providers, and multidisciplinary team
members.
● Ability to maintain professional boundaries and work effectively with individuals from diverse
backgrounds.
● Strong organizational, documentation, and time-management skills.
● Ability to respond calmly and effectively in crisis situations.
● Ability to lift, transfer, and assist clients with mobility needs in accordance with safety
guidelines.
● Bilingual English/Spanish preferred but not required.
