Job Description
We are seeking a Bilingual (Spanish-speaking) Patient Care Coordinator to join our growing home care team in Brooklyn. In this vital role, you will serve as the bridge between patients, families, and healthcare providers, ensuring coordinated, compassionate, and patient-centered care that helps individuals remain safely and comfortably in their homes.

What You'll Do
- Serve as the primary point of contact for patients and their families
- Coordinate individualized home care services based on each patient's needs
- Manage scheduling, follow-up appointments, and in-home assessments
- Educate patients and caregivers on care plans and available services
- Monitor patient progress and maintain accurate documentation
- Collaborate with healthcare providers, home health aides, and interdisciplinary team members
- Ensure compliance with agency policies, regulatory requirements, and quality standards
- Provide compassionate, responsive support throughout the patient's care journey
What We're Looking For
- At least one (1) year of experience in care coordination, case management, patient services, or clinical support (home care experience preferred)
- Fluent in Spanish and English (required)
- Excellent communication, organizational, and interpersonal skills
- Proficiency with Microsoft Office and electronic health record (EHR) systems
- Ability to prioritize and multitask in a fast-paced environment
