Position Description

We are seeking a dedicated and patient-focused Physical Therapist (PT) for a 13-week contract assignment in a Skilled Nursing Facility (SNF) in Sonora, CA. This opportunity is ideal for a clinician passionate about helping patients improve mobility, restore function, and achieve greater independence through individualized rehabilitation programs.

As a PT, you will collaborate with interdisciplinary teams to evaluate patient needs, implement evidence-based treatment plans, and support positive patient outcomes in a patient-centered environment.

• Graduate of an accredited Physical Therapy program
• Current BLS certification preferred
• SNF or post-acute care experience preferred
• Strong clinical, communication, and documentation skills
