Home Health Physical Therapist - $15,000 Sign-on Bonus

What You’ll Do

As a Home Health Physical Therapist, you’ll deliver high-quality physical therapy services while working alongside physicians, nurses, and fellow rehab professionals to create meaningful outcomes for every patient.

Your day-to-day will include:

• Evaluating patients and developing individualized treatment plans
• Delivering hands-on therapy, therapeutic exercise, and functional training
• Educating and motivating patients to reach their mobility and recovery goals
• Utilizing a wide range of treatment modalities (manual therapy, hydrotherapy, ultrasound, traction, and more)
• Monitoring progress and adapting treatment plans for optimal results
• Collaborating on discharge planning and designing home exercise programs
• Supervising Rehab Aides and supporting staff development
• Maintaining accurate documentation and ensuring patient confidentiality
• Promoting a safe, clean, and compliant treatment environment

What We’re Looking For

• Active Colorado Physical Therapy License (required)
• BLS certification (required)
• Previous experience preferred—but motivated new grads are welcome to apply
• Strong clinical skills and a patient-centered mindset
• Excellent communication and documentation abilities
• Commitment to quality care, teamwork, and professional growth

Physical Requirements

• Ability to stand, walk, bend, stoop, squat, and assist patients throughout the day
• Medium physical demand level
• Lift up to 50 lbs occasionally and 20 lbs frequently
• Full range of motion and manual dexterity to operate equipment and technology
