Position Summary

We are seeking an experienced, results‑oriented Licensed Nursing Home Administrator to lead operations for a large skilled nursing facility. The successful candidate will demonstrate strong leadership, financial and operational management, and proven CMS/NYS survey experience to drive quality, regulatory compliance, and staff engagement.

Key Responsibilities

  • Provide overall executive leadership and day‑to‑day operational oversight of the facility.
  • Ensure regulatory compliance and lead survey/readiness activities with CMS and NYS DOH; successfully manage plans of correction.
  • Drive clinical and operational quality metrics (QAPI, CMS star measures, readmissions, infection prevention).
  • Oversee financial performance: budgeting, staffing models, cost control, and payer mix optimization.
  • Build and mentor a high‑performing leadership team.
  • Maintain strong physician/medical director and community relations; lead strategic initiatives and capital planning.
  • Represent the facility to the corporate office, board, and external stakeholders.


Qualifications and Education Required:

  • Minimum 5–7 years leadership experience in long‑term care/skilled nursing, with demonstrated success in facility administration.
  • Active, unencumbered NY Licensed Nursing Home Administrator (LNHA).
  • Proven record of successful CMS and NYS DOH survey outcomes and regulatory remediation.
  • Strong financial acumen, operational management, and staff leadership skills.
  • Excellent communication, problem‑solving, and change‑management capabilities.
  • Minimum Bachelor’s degree, Master’s preferred
  • Ability and commitment to lead, exemplify, and live St. John’s Brand Characteristics

Preferred

  • Experience leading large (200+ bed) or multi‑site facilities.
  • Prior experience with value‑based programs, managed Medicare/Medicaid payer strategies, and quality improvement methodologies (Lean, Six Sigma, etc.).
