Responsible for functioning as a member of the Integrated Care Team in an entity contracted with MassHealth as a One Care Program. To provide an evaluation, independent of the One Care Programs, of the need and eligibility of Members for support and services in the community and act as their advocate as needed on the ICT. To promote the independent living, social model of disability, wellness and recovery models of engaging with disabled individuals. To work to prevent premature institutionalization and help the member identify/attain personal goals necessary to promote their health and well-being and enhance their ability to live independently in the community.

  • Complete member oriented needs assessment which identifies issues or problems that inhibit secure independent living at home as well as LTSS goals of the Member in accordance with One Care program instructions and direction from ICT.
  • Assessing a Member's functional, health and income status to determine eligibility and appropriateness for community long term care services and to ensure members have access to public benefits and other community services and programs, assisting with referrals and applications when appropriate.
  • Working with the Member, ICT, and informal caregivers (when appropriate) to develop a service plan which addresses the Members' needs and identifies goals to enhance independent living.

Travel reimbursement is $0.70/mile

Minimum Qualifications:

  • Bachelor's Degree in related field preferred
  • Associate's Degree with two (2) years of experience working in human service field with disabled individuals OR High School Diploma with 3 years of relevant experience with disabled individuals required
  • Must have valid driver's license and reliable transportation.
  • MA Social Work license desirable.
  • Must be able to climb multiple flights of stairs

Public Service Loan Forgiveness (PSLF)

If you are employed by a not-for-profit organization such as GSSSI, you may be eligible for the PSLF Program for student loans.
