Job Description
Registered Nurse (RN) – Home Health
Location: Chambers & Randolph Counties, AL
Schedule: Monday–Friday | 8:00 AM – 5:00 PM (Eastern Time)
License Required: Active Alabama RN License or Compact RN License
On-Call: One weekend per month
Productivity Expectation: 30 points per week
Mileage Reimbursement: $0.43 per mile
Position Overview
We are seeking a compassionate and experienced Registered Nurse (RN) to provide high-quality home health care throughout Chambers and Randolph Counties, Alabama. In this role, you will complete comprehensive patient assessments, develop individualized care plans, coordinate care across multidisciplinary teams, and help patients achieve the highest possible level of health and independence.
The ideal candidate has previous home health experience, strong clinical assessment skills, and is comfortable working independently while collaborating closely with physicians and other healthcare professionals.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized plans of care.
- Deliver skilled nursing services in the home setting while promoting patient independence and safety.
- Complete accurate and timely OASIS assessments and documentation using HomeCare HomeBase (HCHB).
- Coordinate care with physicians, therapists, social workers, discharge planners, and other members of the healthcare team.
- Manage care transitions and ensure continuity of care following hospitalizations or changes in patient status.
- Identify barriers to treatment and coordinate non-medical resources, such as transportation or community services, to support patient success.
- Monitor patient progress and communicate changes in condition to the appropriate providers.
- Perform skilled nursing procedures including wound care, PICC line management, IV therapy, medication administration, and patient education.
- Promote health maintenance, disease prevention, and chronic disease management.
- Maintain detailed clinical documentation, including patient outcomes, interventions, and care coordination activities.
- Advocate for patients while maintaining confidentiality and ensuring high-quality, patient-centered care.
- Participate in the on-call rotation one weekend per month.
Qualifications
Required:
- Active Alabama RN License or Compact RN License
- Associate's or Bachelor's Degree in Nursing
- Current unrestricted RN license
- Strong clinical assessment and critical thinking skills
- Excellent communication and care coordination abilities
- Current driver's license, reliable transportation, and auto insurance
Preferred:
- Previous Home Health experience
- HomeCare HomeBase (HCHB) experience
- OASIS documentation experience
- Wound care experience
- PICC line management
- IV therapy experience
- Case Management certification
Productivity Expectations
- Maintain a productivity goal of 30 points per week
