Job Responsibilities

• Order Verification: Review, interpret, and accurately verify inpatient physician medication orders in a timely manner, assessing for drug-drug interactions, appropriate dosing, and duplicate therapies.

• Clinical Consultation: Provide evidence-based clinical consultations to physicians, nurses, and other healthcare providers regarding complex medication regimens, intravenous compatibilities, and formulary alternatives.

• Compounding & IV Oversight: Oversee and validate the accurate compounding of sterile products (IV medications, TPNs) in adherence to USP <797> and USP <800> strict cleanroom guidelines.

• Therapeutic Drug Monitoring (TDM): Perform pharmacokinetic dosing and monitoring for specialized medications (e.g., Vancomycin, Aminoglycosides, Anticoagulants) in accordance with established hospital protocols.

Job Requirements

• License: Active, unrestricted Pharmacist license issued by the California Board of Pharmacy (CA RPh).

• Experience: Minimum 1 to 2 years of recent, independent clinical inpatient/acute care hospital pharmacy experience.

• Technical Skill: High proficiency using hospital informatics and automated dispensing systems (e.g., Epic Willow, Omnicell, or Pyxis).

• Life Support: Active American Heart Association (AHA) BLS certification.
