$35 per Visit

What you’ll do:

Provide skilled nursing care and treatments as ordered by the physician

Monitor patient conditions and report changes to the RN case manager and physician

Administer medications and perform wound care and other nursing procedures

Educate patients and families on treatment plans, medications, and disease management

Accurately document patient care and progress

Collaborate with the interdisciplinary care team to support patient goals
