Campbell Hall Rehabilitation Center is seeking a dedicated and compassionate Speech-Language Pathologist (SLP) to join our exceptional rehabilitation team on a part-time basis (approximately 20 hours per week).

Position Overview:
As a Speech-Language Pathologist, you will evaluate, develop, and implement individualized treatment plans for residents with speech, language, cognitive, voice, and swallowing disorders. You will play a vital role in helping residents maximize their independence, safety, and overall quality of life.

Key Responsibilities:
  • Evaluate residents with communication, cognitive, speech, language, voice, and swallowing disorders.
  • Develop individualized treatment plans based on clinical findings and physician orders.
  • Provide skilled speech therapy services that support resident goals and functional outcomes.
  • Perform swallowing assessments and collaborate with the interdisciplinary team regarding diet recommendations and dysphagia management.
  • Educate residents, family members, and caregivers on treatment strategies and home programs.
  • Document evaluations, treatments, progress notes, and discharge summaries in compliance with regulatory standards.
  • Participate in care plan meetings, interdisciplinary rounds, and discharge planning.
  • Communicate resident progress and recommendations to physicians, nursing staff, and rehabilitation colleagues.
  • Maintain compliance with all federal, state, and facility guidelines.

Qualifications:
  • Current New York State Speech-Language Pathologist license required.
  • CCC-SLP preferred; Clinical Fellowship (CF) candidates are encouraged to apply.
  • Experience in skilled nursing, rehabilitation, or long-term care settings preferred.
  • Knowledge of dysphagia management, cognitive rehabilitation, communication disorders, and PDPM preferred.
  • Strong clinical, organizational, and documentation skills.
  • Ability to work independently while collaborating effectively as part of an interdisciplinary team.
  • Passion for delivering compassionate, resident-focused care.
