Location: Grove City, PA 16127

Responsibilities

  • Evaluate residents referred for physical therapy services.
  • Develop individualized treatment plans based on each patient's needs, abilities, and physician recommendations.
  • Provide skilled physical therapy treatments to improve mobility, strength, balance, and overall functional independence.
  • Establish realistic, measurable goals and monitor patient progress.
  • Complete timely documentation, including evaluations, progress notes, recertifications, and discharge summaries.
  • Educate residents and family members on treatment plans, exercises, and safe mobility techniques.
  • Collaborate with physicians, nursing staff, and other therapy professionals to provide comprehensive, patient-centered care.
  • Maintain compliance with HIPAA, facility policies, and all applicable federal and state regulations.
  • Perform other duties as assigned by the Director of Rehabilitation.

Qualifications

  • Graduate of an accredited Physical Therapy program.
  • Current Physical Therapist (PT) license in the state of Pennsylvania.
  • Experience in skilled nursing, long-term care, or geriatric rehabilitation is preferred, but not required.
  • Strong communication, documentation, and interpersonal skills.
  • Ability to work independently while collaborating effectively with the rehabilitation team.
