HOME CARE COORDINATOR

Schedule Coordinator – In-Home Care Services

Position Overview

We are seeking an experienced, compassionate, and highly organized Home Care Scheduler / Care Coordinator to join our team. This position is primarily responsible for scheduling and coordinating home care providers to ensure our clients receive reliable, high-quality in-home care services.

The ideal candidate will have previous home care experience and understand the unique challenges of coordinating caregivers in a home care environment. Success in this role requires strong scheduling skills, excellent communication, the ability to quickly resolve staffing issues, and a commitment to providing exceptional service to both clients and caregivers.

Responsibilities:

  • Coordinate and maintain caregiver schedules for in-home patients
  • Match caregivers with patients based on care needs, availability, location, and skill set
  • Manage daily scheduling changes, call-offs, coverage needs, and shift replacements
  • Communicate with caregivers regarding schedules, assignments, and updates
  • Communicate professionally with patients and families regarding service schedules and care coordination
  • Monitor caregiver attendance and ensure scheduled visits are completed
  • Maintain accurate patient and caregiver records
  • Assist with onboarding and orientation coordination for new caregivers
  • Document schedule changes and follow established procedures
  • Work closely with the care team to ensure continuity and quality of services
  • Provide excellent customer service to patients, families, and caregivers

Qualifications

  • Previous experience in home care, healthcare scheduling, staffing, or a related field preferred
  • Experience coordinating caregivers, aides, nurses, or healthcare staff strongly preferred
  • Excellent organizational and time-management skills
  • Strong problem-solving abilities and attention to detail
  • Ability to remain calm and professional in urgent scheduling situations
  • Strong verbal and written communication skills
  • Proficiency with computers, email, scheduling systems, and Microsoft Office

Preferred Skills:

  • Knowledge of home care operations and caregiver responsibilities
  • Experience with scheduling software or electronic documentation systems
  • Ability to build positive relationships with caregivers and patients
  • Strong customer service mindset
