This position supports older adults by evaluating their needs and linking them with helpful services and resources. You will guide individuals and families through important steps that promote safety and well-being. As an Aging Care Manager 1, you will perform the following duties:
- Care Plan Development: Assess consumer needs and create care plans based on social contact, daily activities, cognitive status, physical health, and available resources
- Information Gathering: Collect details from consumers and their support networks to identify concerns and determine needed services
- Service Coordination: Connect individuals with community programs and resources that address their unique situations
- Income Assistance: Help consumers manage income to support their daily living needs
- System Reporting: Use the State reporting system to enter accurate case information and updates
Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with 30-minute lunch.
- After hours and on-call may be required.
- Telework: You will not have the option to telework in this position.
- Salary: In some cases, the starting salary may be non-negotiable.
- Upon successful completion of the six-month probationary period in accordance with the union agreement, you will be reassigned to an Aging Care Manager 2 position with a corresponding pay increase.

QUALIFICATIONS
Minimum Experience and Training Requirements:
- Three years as an Aging Case Aide 2; or
- A bachelor's degree which includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training.
- Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree/credit.

- Hiring preference for this vacancy may be given to candidates who live within Potter County. If no eligible candidates who live within Potter County apply for this position, candidates who reside in other counties may be considered.
- You must be able to perform essential job functions.

- This position falls under the provisions of the Older Adult Protective Services Act.
- Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
- This position falls under the provisions of the Child Protective Services Law.
- Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
