Work Environment

- All work is performed in offices that require working in close quarters with other employees. The pharmacy settings are air conditioned / heated. \n
- Adequate protective wear available for handling certain drugs (i.e., chemo) such as a mask, gown and gloves. Gloves and goggles are available for other procedures as necessary. \n
- Distribution of Medications – walking, standing, lifting, carrying, pushing, manual dexterity, reading \n
- Drug Information / Clinical Activities – sitting, talking, vision, fingering, manual dexterity, reading \n
- Keyboarding / Computer – Manual dexterity, sitting, grasping, vision, reaching 6” \n
- Answering Telephones – reaching 6”, fingering, hearing \n
- Compounding – walking, standing, lifting, carrying, pushing, manual dexterity, reading \n
- Equipment Used: carts, computer, printer, pharmacy robotics, phones and fax machines \n
- Employees are required to have reasonable access to transportation to meet work requirements made in the ordinary course of business in a timely and efficient manner. \n

Review medication orders for appropriateness (dose, route, frequency, indication, interactions, allergies)

Verify and dispense physician-ordered medications in accordance with hospital protocols

Perform medication reconciliation upon patient admission, transfer, and discharge
\n - \n
Monitor patient therapy outcomes and adjust or recommend changes to optimize treatment
\n - \n
Provide drug information to healthcare providers, patients, and caregivers
\n - \n
Evaluate and prevent adverse drug events and medication errors
\n - \n
Ensure compliance with hospital formulary and therapeutic guidelines
\n - \n
Maintain inventory control of medications, including controlled substances
\n - \n
Document all activities in the electronic health record (Epic)
\n - \n
Ensure compliance with state and federal regulations
\n - \n
Precept pharmacy students and residents in clinical and operational settings
\n
Position Qualifications
\nMinimum Required:

A Bachelor’s of Science Degree (B.S.) or Pharm.D. in Pharmacy from an accredited college or university
\n - \n
Licensed as a Pharmacist in New York State or NYS internship permit
\n
Preferred:

One year experience as a pharmacist
