American Traveler is seeking a licensed Physical Therapist with home health experience for a 13-week home health assignment in Kingman, AZ.

Details\n

• Home health setting serving patients in the Kingman, AZ area
\n
• Patient diagnoses include CVA, heart failure, Type II diabetes, post-amputation, Parkinson's disease, paraplegia, and dementia
\n
• Monday through Friday day shift, 8:00 AM – 4:30 PM (5x8-hour shifts)
\n
• No on-call requirements
\n

Requirements\n

\n
• Minimum 1 year of physical therapy experience required
\n
• Home health or outpatient PT experience required
\n
• Current AZ PT license required (must be in hand at time of consideration)
\n
• Current BLS certification required
\n
• Level 1 fingerprint card required
\n
• Two professional references required for consideration
\n
• PDF copies of driver's license and Social Security card required for consideration
