Chenega Government Mission Solutions, LLC, a Chenega Professional Services’ company, is looking for a Diabetes Pharmacist, Level II, in support of the Lawton Indian Hospital and its satellite clinics: Anadarko and Carnegie Indian Health Clinics. The Pharmacist will provide comprehensive clinical care, diabetes education, and medication management for beneficiaries of the Indian Health Service.

Responsibilities\n

Clinical Care & Patient Education

\n
    \n
  • Provide comprehensive, professional, and patient-centered pharmacy services for pediatric, adult, and geriatric patients, with a strong focus on diabetes management
  • \n
  • Deliver extensive patient counseling to ensure understanding of medications, side effects, drug interactions, and expected outcomes, with emphasis on diabetes self-management
  • \n
  • Evaluate glycemic trends using EHR tools and continuous glucose monitors (CGMs); interpret data to optimize therapy
  • \n
  • Manage and adjust medication regimens under collaborative practice agreements using evidence-based guidelines
  • \n
  • Support medication adherence by identifying and addressing barriers to appropriate medication-taking behaviors
  • \n
  • Perform CGM placement and provide education on use and lifestyle modifications, including basic nutrition guidance
  • \n

Medication Management & Pharmacy Operations

\n
    \n
  • Process, verify, dispense, and document prescription orders, ensuring medication safety and accuracy
  • \n
  • Manage refill authorizations, call-in prescriptions, and patient refill lines
  • \n
  • Oversee medication inventory, including controlled substances, vendor orders, and returns
  • \n
  • Maintain pharmacy automation systems and ensure proper stocking of medications and supplies
  • \n
\n

Care Coordination, Compliance & Quality Improvement

\n
    \n
  • Utilize RPMS and the Electronic Health Record to document care and support continuity of treatment
  • \n
  • Assist with patient flow, scheduling coordination, and communication via phone and fax
  • \n
  • Maintain compliance with HIPAA, The Joint Commission (TJC), and safety/infection control standards
  • \n
  • Support quality improvement initiatives and participate in departmental and facility programs
  • \n
  • Maintain licensure, complete continuing education, and participate in required trainings and safety protocols
  • \n

Qualifications\n
\n
\n
    \n
  • Graduate of accredited College of Pharmacy
  • \n
  • Permanent, current, full and unrestricted license to practice in the United States, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
  • \n
  • Board Certification in Ambulatory Care Pharmacy (BCACP) or Pharmacotherapy (BCPS) from the Board of Pharmacy specialties OR completion of a Post Graduate Year One residency.
